What kind of supporting documentation do I need to submit for reassignment under the hardship category?

To be considered for a hardship reassignment, you must provide the Hardship-Student Reassignment Request Form (H-SRRF) along with specific evidence that substantiates your unique situation.

Supporting documentation varies based on the nature of the request:

  • Mental Health or Medical Hardship: A completed Recommendation Form from a licensed healthcare provider or mental health professional. This must include a diagnosis, treatment plan, and a clear explanation of why a school transfer is medically or therapeutically necessary.
  • Student Conflict or Bullying: Evidence of documented incidents and prior attempts to resolve the conflict at the school level. This may include a Principal’s Official Complaint Outcome Letter, police reports (if applicable), or a copy of a filed Bullying, Cyberbullying, Harassment, or Intimidation Reporting Form.
  • Other Unique or Family Hardship: A detailed narrative from the parent/guardian and official supporting records, such as legal records, military orders, or other verifiable documentation of the unique hardship.
  • Evidence of Prior Resolution: In all cases, you must provide documentation showing that reasonable efforts were made to resolve the concern with school-based staff and that those efforts were unsuccessful.

Note: Please do not request letters of support from school-based staff. The Office of Pupil Personnel Services will contact the school directly for feedback if appropriate. 

Submitting documentation does not guarantee approval, and additional information may be requested during the review process.
