What is the Notice of Telehealth Consent form?

The HCPSS Notice of Telehealth Consent form allows a related service provider to practice in a virtual environment. All related service providers must follow federal and state regulations with regard to privacy and security via telepractice in order to maintain their national certification and licensure.
 
Service providers must receive parent/guardian consent for telehealth services and include it in the student's medical record. Parents/guardians will receive the form from the service provider, and also may give verbal consent. Consent is strictly for the service to occur virtually. It is not related to a student's IEP goals and objectives or to the IEP itself.
 
The consent form provides parents/guardians with information about the requirements involved in a telehealth session, expected benefits, possible risks, limits of confidentiality and ways to provide informed consent.
