What should a staff member do if they are eligible for an accommodation through the Americans with Disabilities Act (ADA)?

Staff with a documented disability may qualify for accommodations. All applications for ADA accommodations are considered and decisions are made based upon the facts of the case. Please note that the age of the employee and disabilities of family members are NOT factors that qualify for ADA accommodations.

Some staff will qualify for an accommodation of continuing to work from home. This includes staff unable to be vaccinated for medical reasons and staff who have conditions defined by the Centers for Disease Control and Prevention as being at an increased risk of severe illness from the virus that causes COVID-19.

To request an ADA accommodation, please have your medical provider fill out and return this form. Visit the U.S. Department of Labor website to learn more about accommodations.
